In a completely diferent sound master volume thingy (SMVT, that's oh so
technical!), I had to start each of the programs while the SMVT was running
so I could set defaults per program. e.g. I couldn't see Firefox unless it
was started after the SMVT was already running.

Did you try pavucontrol? I found out for me (on Debian Buster) that
using "Custom" for the master channel and adjusting the volume for the
specific source (Skype, Firefox) in pavucontrol solves the "low volume"
or mitigates it.
