I have used KDE3 and KDE4 themes. I just download and install as appropriate via the relevant configuration tool. Accessible through KControl. You don't say what you are trying to adjust. One gottcha I hit: don't unzip a downloaded mouse theme.
I could give more detailed instructions if I knew what you were trying to change. I have personally recently downloaded and installed mouse themes and wallpapers/backgrounds.
