Toolbar customization is a little bit of a pain because of the structure of Kate. You can do some customization but do not have full control as in some other editors. The reason is that the toolbar displays a collection of toolbars: there is the main one, the KPart one and some others. They merge together at runtime following a somehow cumbersome logic and so for example it is not always possible to place a given button exactly were you want. Sometimes you need to compromise. Also note that in some cases you make a change to the toolbar buton order but nothing seems to happen. Then you close and reopen Kate and the order is now what you wanted (yes, there is already a bug open for this).
BTW, in case you have been using Kate Tab Bar extension for years, you may have missed that since a couple of years ago middle-clicking the button in the tab bar closes the document, which is quite handy when you get use to it ;-)
