usually in .Xresources, the resource is called "Xcursor.theme".
e.g:
$ xrdb << XXX
Xcursor.theme: a-theme-name
XXX
sets the theme from /usr/share/icons/a-theme-name/

It appears not to matter actually - it always uses that cursor without
either command. I suppose that it is just a default somewhere.

Does the xserver of your mac honor X11 cursor themes?

I think it must because it is getting set somehow. by default it uses
the regular mac one until set otherwise
