Recommended upgrade path?

I have a system at home running Kubuntu 10.10 (maverick) running Trinity before it was actually Trinity (3.5.12 - packages are all *-kde3). What is the best way to update this to a more modern version of Ubuntu, say 12.04 LTS? Same question for my work system, but it's at 11.04/Natty Narwhal with Trinity 3.5.13.
